Hormone Management

Testosterone Therapy 

A woman’s testosterone levels naturally change throughout her life, her menstrual cycle, and even at different times of the day. A woman with low testosterone does not contain enough to help produce new blood cells, maintain sex drive, or boost levels of other reproductive hormones.

Testosterone belongs to a group of hormones known as androgens. Testosterone levels affect:

  • fertility
  • sex drive
  • red blood cell production
  • muscle mass and fat distribution
  • Most people think of testosterone as a male sex hormone, but everyone requires a certain amount. While males have more testosterone than females, female adrenal glands and ovaries produce small amounts of this hormone.

Low testosterone can cause one or more of the following symptoms in women:

  • sluggishness
  • muscle weakness
  • fatigue
  • sleep disturbances
  • reduced sex drive
  • decreased sexual satisfaction
  • weight gain
  • fertility issues
  • irregular menstrual cycles
  • vaginal dryness
  • loss of bone density

The two main causes of low testosterone are:

  • diminishing levels of the hormone as a normal result of menopause and aging
  • problems with the ovaries or the pituitary or adrenal glands.

Testosterone decreases naturally as a woman ages. Levels of other hormones, such as estrogen, also reduce over time, especially when a woman reaches menopause.


Around the time that menopause begins, a woman may be more likely to have less testosterone because the ovaries are producing fewer hormones.


Also, medications that combat the side effects of menopause can lower testosterone levels. One such medicine is oral estrogen.


Problems with the ovaries and adrenal glands can also cause lower levels of testosterone. A woman may have reduced levels if her ovaries have been removed, for example, or if she has adrenal insufficiency, which means that the adrenal glands do not work correctly.


Treatment

A doctor may administer testosterone injections or pellets, expecting these treatments to have the same effect on women as on men: raising energy levels, decreasing fatigue, and increasing the sex drive.

Vaginal PRP Therapy

According to the American Heart Association, 50% of men and 64% of women who die suddenly of Coronary Vascular Disease have no previous symptoms. Early detection is crucial for proper treatment for sustained recovery.


There is an approach that detects meaningful disease and allows for monitoring efforts to address the hardening of the arteries. It is non-invasive, takes about 15 minutes, and is covered by most insurance companies.


It will tell you whether you are forming plaque, the lesion that is present in almost all heart attacks and strokes. And it is used in research to assess the effects of treatments offered to treat vascular disease of the heart, brain, and other organs.


This approach is called Carotid Intima Media Thickness scanning. We offer it to determine which patients need more proactive measures to PREVENT heart attack and stroke. We use Vasolabs, a provider whose commitment to consistent process gives me the confidence to rely on their results in making treatment recommendations. It is an enhancement of carotid duplex that provides more information that affects efforts to slow or regress atherosclerosis (hardening of the arteries).

Cardiac Risk Assessment 

Cardiovascular disease refers to a number of conditions related to the heart or blood vessels. These include hypertension (high blood pressure), coronary artery atherosclerosis (build-up of fatty deposits in blood vessels supplying the heart), cerebrovascular atherosclerosis (buildup of fatty deposits in blood vessels supplying the brain), and peripheral vascular disease (blockage of large blood vessels in the legs or arms), to name a few.


Such conditions, if not properly treated, may lead to a cardiovascular event such as acute heart failure, myocardial infarction (heart attack), angina (chest pain), thrombosis (abnormal blood clot in a blood vessel), or cerebrovascular accident (stroke). Other factors, such as smoking, obesity, and family history, can contribute to the risk of a cardiovascular event.


We have joined with Boston Heart Diagnostics to provide a Cardiac Risk Assessment. Boston Heart Diagnostics uses proprietary blood tests, available only from them, such as the Boston Heart HDL Map, the Cholesterol Balance test, Advanced Risk Markers, Genetic Tests, as well as the metabolism of glucose (sugar) in your body, all of which may affect your heart health. Endometrial Biopsy 

The endometrial biopsy is a medical office procedure that is used to remove a sample of the lining of the uterus. The tissue subsequently undergoes a histologic evaluation which is supplied to Dr. Marden to aid in a diagnosis of symptoms which can include abnormal bleeding and infertility issues.


An endometrial biopsy involves inserting a thin, flexible tube through the cervix and into the uterus. The tube is used to gently scrape a small amount of tissue from the endometrium and then remove the tissue samples for evaluation.

Colposcopy 

If you have had a Pap smear that resulted in abnormal results, the next step is usually a colposcopy.


Colposcopy is a simple, 10- to 15-minute procedure that is painless and is performed in our office. You are positioned on the examination table like you are for a Pap smear, and an acetic acid (such as common table vinegar) is placed on the cervix.


Dr. Marden will use a colposcope — a large, electric microscope that is positioned approximately 30 cm from the vagina — to view your cervix. A bright light on the end of the colposcope lets Dr. Marden clearly see the cervix so he is able to select the best area for a biopsy. If abnormal areas are identified, a sample of the cervical cells is taken and submitted to a laboratory for evaluation.

Intrauterine Device (IUD)

Dr. Marden will discuss the pros and cons of these options with you so that you can determine the best birth control method for your situation.


ParaGard is an intrauterine device (IUD) for birth control that does not contain synthetic sex hormones. The ParaGard IUD is made of flexible plastic and copper, crafted into the shape of a small “T.” It is a little over an inch long, with fine copper wire wrapped around the stem and arms. ParaGard does not contain latex. The IUD must be placed inside the uterus by a doctor, where it may remain for up to ten years. ParaGard starts working as soon as it is inserted into the uterus. The IUD works continuously without having to take a pill.

Mirena is an intrauterine (IUD) contraceptive that delivers small amounts of hormone directly to the uterus. Made of soft, flexible plastic, it is put in place by your healthcare provider during an office visit. Mirena is birth control that lasts as long as you want, for up to 5 years, and is also approved to treat heavy periods in women who choose intrauterine contraception. Mirena works continuously without having to take a pill.


Skyla is a hormone releasing IUD that is highly effective in preventing pregnancy for up to three years. It is a small flexible plastic t-shaped device that is placed in the uterus by your doctor in his office.

Ultrasounds 

Gynecological ultrasound imaging is useful to help detect and diagnose any fertility or reproductive issues as well as provide additional information to resolve issues of pelvic pain and abnormal vaginal bleeding.


A typical pelvic sonogram requires fewer than 30 minutes. However, the length of time may vary greatly depending on how easily the necessary information is obtained and on the complexity of the findings.
